PROJECT
I need to build an online survey. it will contain 2 sections.
First section has 14 questions and it will require user to move 4 words across from left column to right column and place those words in order of preference.
Second Section contains 12 questions and it will require user to move 4 sentences across from left column to right column and place those sentences in order of preference.
If drag and drop is too difficult and time-consuming to build, I am open to building the survey with a simple way of respondents answering each question with an order of preference i.e. “1. Active 2. Passive 3. Happy 4. Sad” without the respondents having to write the words, but rather check them or number them.
I would also like for people to include name and Em address of each person when beginning to fill out the form.
I am open to your suggestions.
I will also need the data on a spreadsheet with name, Em address and results.
I want to own this survey and embed it in a website.
Please advise if you can do this task and details - cost and when you can deliver it. I have all the content ready to go.
