Comments Script (Small Script)

I'm looking for a simple comments script. Which allows me to add a comments box to ANY webpage, using php.
Something similar to this demo: [url removed, login to view]
I would like it coded in Ajax and without the need of a mysql database.
It needs the following features:
Front-End -
* I would like the same look as the example above (default)
* the form needs:
- Name~
- Email~
- Website URL
- Title
- Comments~
- Captcha image~
(~ required fields)
Admin area -
* view/edit/delete comments
* ban IP
* enable/disable form fields: website url & title
* comments editable template (change look of front-end)
So just to clarify, the script needs to run without a mysql database. And the script needs
to give the admin the option of allowing visitors to leave either separate comments for
each separate webpage or just one set of comments for all pages.
I need the script asap, so I would like the project completed within 3-4 days.
