Hello!
I need a custom form that allows users to input text, select categories, add tags, upload images and videos. The user can also send the admin a note.
This custom form also has an admin page where the admin can login and inspect submitted forms, and either approve or decline them. A note can be included to the user that submitted the form.
The form the user fills out has the following on it:
4 TEXT box categories:
Short title: Text box minimum 20 characters max 40
Long title: Text box minimum 20 characters max 70
Short description: Text box minimum 50 characters max 70
Long description: Text box minimum 50 characters max 100

1 TAGS box
Tag text box goes under this, this text box must automatically re-arrange any words in here in this format:
word,thing,place,dog,cat,etc
This text box is comma delimited and simply labeled "Tags." Maximum of 20 tags. NO spaces between words allowed.

1 STAR box
Please add another text box called "Stars" with it again comma delimited but allow for spaces in the words. SPACES ARE ALLOWED between words, example:
name name,barrack obama,hillary clinton,michael jackson,

There are 6 drop down boxes that must each be unique when user is filling out the forms, and they are labeled Category1, Category2, Category3, Category4, Category5, Category6

Please allow the following menus to allow "browse" to choose the following files:
Box 1 "pic"
Box 2 "pic"
Box 3 "3 second big gif"
Box 4 "3 second gif"
6 MORE GIF BOX Add a space separating these next set of 6 boxes
Box 1 "1 second gif 1"
Box 2 "1 second gif 2"
Box 3 "1 second gif 3"
Box 4 "1 second gif 4"
Box 5 "1 second gif 5"
Box 6 "1 second gif 6"
2 MORE VIDEO BOX Put another space here and now we need to allow for video uploads
Box 1 "10 second HD clip"
Box 2 "5 second HD clip"
3 MORE VIDEO BOX Add a space separating the above movie clips with the next set of boxes
Box 1 "3 minute or greater sponsor clip 1"
Box 2 "3 minute or greater sponsor clip 2"
Box 3 "3 minute or greater sponsor clip 3
All of the text, drop down selections, images and videos need to be saved somewhere, prefer into a wordpress database so my network of blogs can all access these files. Maybe it can be FTP'ED to server?
I need the code to have notes and be written cleanly so I can have you or someone else add to it as my needs from this form grow and change with time!
