Database to form merge and save, MS Access

Coder will create a database application for a nonprofit donor driven organization. Approximately 10-15 fields will be collected for each donation, then with a button press will be merged into a "Thank You" letter, printed, and automatically saved with a file name similar to [url removed, login to view] in a default folder.
Ideally database should be searchable by name OR date OR item, and viewing all activity by one donor should be possible. (Example: one donor makes 10 donations across the year, all should be visible on one tab. Simple sub-form is fine for this.)
## Deliverables
Final delivery is acceptable via .zip or .rar.
As stated, we are a not-for-profit organization so we can't afford very much. I would design this myself if I thought I had the time.
This can be as simple or as grand as someone wishes to make it. It would be great for the database to automatically backup upon close (or at least present the option).
I will provide fields for database to prospective bidders upon request.
