Appointment reservation system

I need a simple reservation system (php) for 2 hypnosis sessions (regular and VIP). The reservation system must fit in a 350 px width div and must contain the following items;

frontend:

- Date selector (visible available timeblocks / days

- Session selector

- Name, adres, zip, town, telephone, emailadres

- captcha

- Payment (cash of in advance)

i also need a backend with secure login:

Backend:

- Date and time blocker (appointments, not available, free etc.)

- Price settings

- Appointment settings

- Email functionality (confirming reservation etc.)
