Assignment 1
This assignment is designed to give you opportunity to install, use and develop an application which uses server side scripts in PHP to access and update an SQL database. The following tasks guide you through the stages involved.
Task 1 Install and run the Survey System. (Done already)
Task 2 Add an additional admin user and supply username and password (already done)
Task 3 Design and enter some questions (at least 5 ) of your own choice (already done)
Task 4 Modify the code to personalize it. Enhance the user interfaces (Web Pages and forms) using html tags and attributes, to change background and foreground colours, fonts, add appropriate images etc.
Task 5 Add some additional functionality to the site using the Php scripting language and SQL. You may consider creating an additional report, that displays information from a table within the database or perhaps adding / modifying a form, to capture information that is then stored within a table. Feel free to modify the database by adding new fields or tables.
Task 6 Using the notation taught, draw a block diagram of the site. The diagram must include the main components of the system (php / html files, web pages, forms etc ) and show how they interact with one another. A short written description of the functionality contained within each script should be included.
Task 7 Produce a test plan with suitable data, enter the data and show results match the expected results of the test plan. While no errors have deliberately been introduced into the site, there may be scripts that do not function as intended. If your testing does highlight a bug, include a brief description of it.
Task 8 Write a report reviewing the survey system and suggest what additional functionality would be required to turn it into an online assessment system. You may like to include extracts of code that demonstrate in part the modifications required. Approx 500 words + code.

Task 8 = Comprehensive and accurate explanation of additional features for on-line assessment system:

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
