Develop Application (mobile? html5?) for use by insurance agents

I need to develop a prototype product with limited functionality. It should be working perfectly fine on mobile platforms. The application will ask user series of questions and based on the answers display the number of insurance companies. Here are the main features:
1) Company set up. Insurance agency sets up the companies they are working with out of the list provided and set up the underwriters, rates etc for each company
2) Risk Identification. Insurance agent picks up the trade (let's say it's a restaurant) out of the list. The app generates trade specific questions i.e how many liquor sales do you have vs food sales. Agents fills out these questionnaire and clicks submit
3) Based on answers in 2. (for example the user indicates that restaurant sells 25% of liquor), the number of insurance companies show up. Some companies in a column are those that like to insure restaurants with liquor sales, some are those that dont and they will have a little bubble with the question mark next to them. In that bubble there will be a preset explanation why the risk is ineligible
4) Now user can send the "submission" to the companies that "like" this business. He goes through another set of questions, these questions are just underwriting information and at the end we generate a pdf application
5) This pdf application is emailed to all eligible companies (their emails are pre set at the setup page).
