Access Database for doctors office. Queries and forms help!

Need very simple work on a simple MS Access database. The DB will be for a doctors office. I have the tables and the forms done. I just need some help with the queries (and adding it to the form) and creating reports. Also might need to convert this to a stand alone DB. More info via PM or as and when needed.
I have the form setup - so that the end user can enter information in to the database. However I will need a query setup wherein when the last name and/or database is entered the information shows up and then the user is able to print the same.
Very simple and should not be very time consuming at all - Shouldn't take more than an hour to complete.
