Corporate logo and banner designer required

We need some artwork done for a web banner (approx 590x120) per our corporate brand guidelines, but with creative application. We may require up to 3 different sized banners.
Ideally 1-2 concepts will be presented, with multiple rounds of changes likely. The right designer for the job will be creative and reliable with excellent client management skills.
Layered artwork in illustrator/ Indesign or Photoshop would also be required (so we can make any tweaks in the future). CS5 ideally.
We are open to various style treatments - and are open to animated, photo etc - but do not want clip art
If you are the best person for the job and can fulfil all the requirements, send us a bid.
Our budget is $200 max (all equivalent design hours incl changes would need to be within this budget)
