Create a Form in Formidable Pro on Wordpress

Hey Guys - We need someone to turn some of the forms my client uses offine into online forms. We use Wordpress and have a Pro subscription to Formidable, so this is the form software we would like you to use. The electronic forms should be laid out in a similar fashion to the offline forms. Once a user fills the form out, it will be emailed to my client, who will receive it in a similar format to how its currently laid out. I've attached a Photo of the form we want you to recreate. We have several other forms too, so there is the opportunity of extra work. Need work done ASAP.
