Copying information from scanned documents

We have around 1600 scanned documents that were completed by hand with simple information like first name, last name, birth date, phone number and email.
We need someone to type that information on a simple web interface that has been setup for this. Please see a screenshot of it attached.
The information has to be typed in only if the document is completed with an email address.
