You must need to be expert on HTML PHP AND CSS as well as JavaScript. basically am using wordpress and I need you to generate a form to capture lead of my customers.. #1 - on top of the page there will be 2 dropdown.. one category and another one sub category. subcategory dropdown will appear after choosing category only.. and the look will be very similar to this one - [login to view URL] as soon as the user choose subcateogry the page need to auto scroll at the middle .. because at the middle of this page we are going to add the form. The form need to be look like same @ - [login to view URL] option choosen on sub category need to place autometically on What you need. user will fill rest of the form and submit. after click on submit.. a new page get loaded and email should send to the admin with from details. #### Remember.. am using VisualComposer to design this page. so I need the code only.
