Wordpress plugin needed

I am in the process of creating a video flash player with the same functions of the one on YouTube. I am in need of a custom WordPress(blogging software) plugin for my website that will load a video profile within a wordpress post. These type of posts will be placed in my "Videos" category and organzied in my sub-categories. This html designed platform will be different from posts another categories such as articles. I want to have a separate plugin tab displayed below the post tab that has input forms for the following: - Turn video platform on/off option - Video Title - Displayed on Page (Separate from Wordpress' actual post title) - Video Description - Displayed on page - Permalink input form - Displayed as a textarea form on the page - VIDEO FILEPATH Input (Example: [login to view URL]) - *This will display the video in it's own zone of the page as well as display the embed code for viewers to select in an textarea form on the page.
