I have a website that needs a form on the first page to give delivery quotes. There are 6 main choices of items, and some choices have a sub categories. The user will enter collection address and delivery address, then submit. Upon submit, they will get a quote price based on the item and mileage. They will then confirm collection/delivery from ground floor/1st floor etc, and when they want the job doing (date/ASAP/Within a week/flexible) This is to be used with a free theme, so the info will have to be automatically forwarded to create an advert.

Pl share algorithm for calculating quote along with the form fields, options. Pl explain what you mean by "forwarded to create an advt".
