Data Entry, Using Ninja Forms Conditional Logic for WordPress from Excel File

We have a WordPress site that has a section for members where they can put in the car make, model, year and cc (engine size). We are using Conditional Logic from Ninja Forms and would like for you to enter the values from an Excel file (attached). This is an urgent thing due by end of day Sunday 21 January 2018. Please prove you can do the job by entering values for at least 3 cars using values from the Excel file.
