Student tracking system for counseling company

We are a college guidance counseling company that is currently using a third party system for our student tracking. We want our own simple, private database for tracking students. It should have the following qualities: 1) A primary login page (with a section where admins have the ability to set up new user accounts) 2) A form for data entry with text fields and checkboxes 3) Spreadsheets, linked to the form, that update accordingly 4) Storage for documents (word docs and PDFs) – file locations also linked to form/spreadsheets 5) The ability to export all spreadsheet data into a CSV, preferably with a single command or click The front end should be an easy-to-use login page and form where counselors can add, update, and download data and documents. An administrator should be able to create/approve/delete user accounts. It is essential that the documents be secure and password-protected. We want to store this in a private folder on our website (hosted by [login to view URL]) Data we want to include on the forms, store on the spreadsheets, and have the ability to download: 1.) Last name, first name 2.) Date of birth 3.) Address 4.) E-mail address 4a.) Phone number (mobile, home, fax, pager -- let's us list five of them) 4b.) A place for us to note the easiest way to contact them 5.) Common app user name 6.) Common app password 7.) Other applications’ username/password 8.) How many sessions did they sign up for 9.) How many sessions did they complete 10.) A place to upload the current draft of: main essay, short answer, a .pdf of the Common App 11.) The college list with a place to upload the relevant supplements and the ability to check safety, realistic, reach 12.) Each session’s notes 13.) SAT scores 14.) ACT scores 15.) SAT Subject Test scores 16.) AP scores 17.) IB scores 18.) GPA 19.) Rigor ranking of courses, with 5 being most demanding, according to the rubric on the SSR 20.) A place to attach the resume 21.) A place to attach recommendations and other documents 22.) A way to record how they came to us (referral, web, etc.) 23.) A name for the type of SAT (etc.) tutoring they use 24.)
