We require a PHP / MySQL database for our client. The database will be used to enter data for their "service" jobs, and will be hosted on a remote server. The budget is low, so we would prefer if you could modify any existing PHP scripts that you may have. There should be a "login" provision with the ability to add new users. Only the administrator has access to the "users" page and can add or delete users if required. A data entry form with the following fields would be required:- 1) Job Number:(Allow 15 Characters) 2) Date: (Format: DD/MM/YYYY) 3) Ships Name: (Allow 50 Characters) 4) Customer: (Allow 50 Characters) 5) Job Description: (Allow a minimum of 500 Characters) 6) Comments: (Allow a minimum of 500 Characters) 7) Date Required: (Format: DD/MM/YYYY) 8) Date Delivered: (Format: DD/MM/YYYY) 9) Agents Details: (Allow 50 Characters) 10) Report 1: (Attach File 1 - This will be PDF or JPG format) 11) Report 2: (Attach File 2 - This will be PDF or JPG format) For "Customer" field, a drop down box would be preferred, however an option for adding a "New Customer" should be provided. For "Report 1" and "Report 2" fields, this should provide the option to upload the scanned copies of the service reports. These files will be in either PDF or JPG format and should be stored on a folder on the website. A "Search" box should be provided for users that are "logged in". These users should be able to search any of the above-mentioned fields (i.e. Job number, Customer, Date, etc) An additional search box should be available on the "Login" page, where customers are able to search without being logged in. They would need to provide the "Job number" and "Ships name" in order to gain access to ONLY the page for the job that they have entered. They will then be able to print the completed form and download the attached "service reports" if required. They should not be able to make any changes or deletions.
