Adding new field to registration form

This is a very simple project. I need an additional field on the registration form for an OsCommerce powered site. This project must be completed as soon as possible, and I know that it's a quick job. You can see the registration form [here][1]. The name of this field is "RST/PST Exemption Number", and it will be located under the "Company Name" field in the "Company Details" section. This field is optional. Underneath this field, in an 8pt font, must be the text "Required for Ontario Businesses". Also, please make sure that this field is stored on the customer's information in OsCommerce and that it is included in the automatic alert email that is sent when a registration is submitted. I'll provide logins to the selected bidder. Please do not bid if you have a poor feedback rating (or no feedback at all!). Thank you! ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform Add on to existing site.
