Hello,

We are looking to develop a similar form to that of [url removed, login to view] which will need to be incorporated into our site at www.elikaassociates.com. The form will need to be built within our current header and footer and in the css styles like on our site perhaps a good reference would be our contact form at; [url removed, login to view]

When the form is complete it must email the inquiry to an email address we will provide and send an automatic email to client that we will send you text for. The form will also need to goto a thank you landing page once form submitted. Looking at finishing this asap.
