My registration website of one file with admin section

OBJETIVE: need a small website with a public html where my visitors to register for my events. when they complete the registration form, saves data to a myql and send the visitor an email of thanks. From the field of administration should be able to view tables, export and print in two versions. maybe in pdf a print version to print cards that will be used during the event and a version for the diploma when the event ends, always using the admin area ----------------------- Please do not use a full CMS ... we need just few files , well documented and /js /css style, no cms already created please.. Files: [login to view URL]: Visitors will complete a form , and after that all data will be stored in mysql, and an email will send to visitor thanking about registring. Forms should have [login to view URL] Thats all for end user. 2-Administration (back-end) We need a backend admin , similar to phpmyadmin to manage database with 3 tables, seeing data and editing it. (style like [login to view URL] ) tables: Visitors, Events and Asistencia Admin can : (1) add new events, edit all visitors details . (2) You need to create a button from this backend after selecting records, generate a pdf to print with this details.: Admin can select all records or some records (like selecting from phpmyadmin skin ), and generate two pdf to print labels or diplomas with visitors details, just using fields in database, no design at all.
