Configure Cisco SG200-26 Switch

Hi Guys,
This project is loosely related to 2 other projects I have live at the moment, and I may award the others to the same person, but since the jobs while similar do not exactly cross over with skillsets, I decided to create multiple projects.
On Tuesday this next week (Jan 12) I have a new Cisco SG200-26 being delivered to the datacenter, and I need it configured to perform a few specific tasks as detailed below. This should be a fairly simple configuration, but please let me know if I Am wrong about that (I am not a cisco tech, so obviously may not understand what all is involved).
I have 5 subnets that are delivered in 4 network drops currently, and I need this switch configured to take those 4 drops in and tag their data with VLAN tags and output those to 6 ports of the switch (one port will connect to each of my 5 servers, with one port spare for my next server), allowing each server to receive all 5 subnets with a single RG45 cable.
In addition, I need 6 ports set up for a management network ([login to view URL]), and 6 ports set up as a private data network ([login to view URL]). I believe just isolating those ports will do that? Not sure. As I said im not a cisco guy lol. I just need it to "work".
I will do all of the non-switch based stuff, such as splitting out the VLAN tagged data on the server to create 5 virtual network adapters so that the servers themselves can access each subnet, etc. This project is simply to configure the switch.
For the time being I will arrange for the 4 subnet network drops to be placed in to ports 1-4, and we will use ports 7-12 for relaying the VLAN tagged trunks to the servers. Ports 13-18 will be for the [login to view URL] network, and 19-24 will be for the [login to view URL] network. At the time of configuration, I will plug a single server in to ports 7 (for the trunk), 13 and 19, for testing purposes. Once the switch is fully configured and tested, I will arrange for some network downtime on my servers and get them moved over to the new switch.
The person awarded this ticket should remain available for assistance during that transition in case of issue, but I understand that additional pay will be required if help with that is necessary - so please bid only on the initial configuration, and any additional work needed will be handled when the time comes.
Bidding on this will close after the weekend, so that things are ready and waiting for the switch when it arrives on Tuesday morning.

This is can be done, would be able to provide a quick network digram as it will be easy to understand and deploy. What are the time frames, that the configs and support needs to happen?

Can deliver the requested configuration either as a text file or remotely on the switch. You just specify the port numbers and the function as per your description.
If the switch has console access remotely this can be done will need the various 4 vlans information with subnet information and if these vlans already have a gateway the configuration of that device would be helpful.
