Need an HTML page that calls an API that returns a JSON object from an HTML template

I usinga Creative Tim template [login to view URL] I have created a trainee-page that I've attached both the static page and the page with the dialog. I need for a person the click the add trainee and add business manager buttons and have the dialog display in a static manner. I need the end user to type a last name in the input box and either click the search button or hit enter. The javascript function will be called and an ajax call to an api will bring back a json object. I need the data to be displayed in the dialog so the end user can select one and only one name. The look up is going to a web api and brings back all names that have the last name in it. Like: [login to view URL] I need the names displayed so the end user can select the name that they want. Very similar to an email program. Once the name is selected it is added to the main table on the main page. That table should also be created dynamically. The page that I'm using is attached as the [login to view URL] and is a clone in many ways of the profile-page Also I need you to find how the image on the background is being sized because I need to reduce the height.
