Convert .doc audit form Into iPad App

Dear Freelancer, We have an auditing form currently in MS Word doc. Please see the attached file that provides a sample of the audit pages, the whole document is currently 14 pages. We need this to be made into an application that can be used run on an iPad or converted into a form that can be used to collect data - the word doc form also needs to be converted into a CSV file. We want to stop using paper forms and want the iPad completed. Output would allow the output to exported, be in a CSV dump of named value pairs for each field of data collected, xlm format and provide a pdf printout. All fields will have a unique field. Either needs to be able to be emailed or FTP'd to a specified location. This data needs to be able to be saved locally incase the iPad is not in range of 3G network, it should appear visually appealing and easy to navigate and use. Please suggest what you would use to for fill this process. The whole process will be conducted by the winning party this involves. Regards, Matthew Product Manager
