We have about eight forms (8.5x11 letter size) and some are double sided.
What we are looking for is a program where we will enter the persons information:
Account Number
Name
Date of Birth
Age ( calculated by the program)
Date of Service (usually today's date)
Name of Insurer
ID Number
Primary care provider
Phone Number
Fax Number
After the above information is entered, typed in, the pre-designed blank forms can be selected (only one... or two... or all forms)... and then printed with the person's selected information for each form on its header and/or required fields.
The information is entered all together and the program will populate the select form(s) to be printed with its required information.
The forms design must be part of the program as it will be printed on blank paper. Some are double sided. See Forms Attached
A data base file with all names entered (each person is a separate record) must be kept and able to have new records added daily... Edit the records for any correction... or have any record deleted.
For recurring clients, the stored information would be just retrieved (updated if necessary) and then printed on the selected form(s).
The way I foresee the functionality is:
Activate the program it it will allow the information to be typed in. Then a choice of which forms to be printed. Then store the typed in information creating a client record. Then the printing of the selected forms. Then back to the beginning for the next client.
No database is really required but one file with several records of each client
The main need is just to print the forms with the basic info of the client already on it.
Don't know what skills are required but a programmer who have some experience.
