single page form in PDF/WORD needs to be in html - repost

I have a single page document in word format and PDF format which needs to be in html format (30 - 60 minute task.)
The idea is it will open in a browser to the same design as is shown in the existing document.
A few key points:
A4 size: 210mm wide x 297mm deep.
the box at the bottom will represent the label, this measures 160mm wide x 105mm deep.
There is a 25mm space at the left and right of the box/ label and 8mm from the foot.
The top of the box/label is positioned 184mm from the top of the form.
