Set Up 4 Mail Chimp Automations + Sign Up Form (Basic)

Hello! I need someone to help me create three basic automation in Mail Chimp based on the information entered into a very simple Sign Up form. When a new customer comes in to see us- we will manually enter into a simple sign up form the following information: 1. First name 2. Email 3. Date of Birth (DD/MM) 4. Are they Pregnant? YES or NO 5. If YES: Due Date of Birth (DD/MM/YYYY) 6. Last appointment date (DD/MM/YYYY) Then from that information- I need the following automated emails to be set up ( can create the design later... I just need it set up properly for me like the merge fields and tags) 1. We Miss You Email : to be sent 3 months after their last appointment date 2. Birthday Email: 1st Email: 2 weeks BEFORE their birthday 2nd Email: on the day of their birthday 3. Welcome Email : 1 day after being added to the list 4. IF yes to Being Pregnant- base around due date: Pregnancy Emails: 1st Email 4 months before due date 2nd email 3 months before due date 3rd email 2 months before due date 4th email 1 month before due date 5th email 1 week before due date If this is something you can do easily and have experience with, then I'd love to hear from you. I really want to make sure it's set up properly. Thank you!
