Custom Wordpress Register Form + Email

This will be quite a unique Registration FORM for my Woo-Commerce Wordpress site. Please have Woo-Commerce experience, as the Woo-Commerce plugin creates the Registration page by default. We need to add to the existing My-Account registration/login page. Ultimately, I need users to register with the following fields: -First & Last Name: -Email (to be used as the username): -Password: -Phone Number: -Upload Document 1(jpg, gif, png) -Upload Document 2(jpg, gif, png) NEXT >> then Clicking Next would bring you to the second half of the form -- but also would send an email to me/staff with all the users submitted info including attachments... The second version of the form is fairly simple - just adding a javascript ECHO-SIGN form that already performs the way I need it to. Literally, just copy and paste the script I send you for the second half of registration. Please send me previous examples of FORMS you have created, if any. Examples highly recommended. In order to start - I will need your IP address once awarded with the job because the site is currently under construction as we finalize the last few details. Will also give you FTP access and WP access, but will also need your IP address.
