Design a Website Mockup for a Chiropractor Business (CRO)

I am looking for someone to design a mock up of a high converting site for chiropractors to get customer appointments. I am looking for people that can show that they have produced results. If you are capable please insert your favorite food into your application (I know it's hoakey). I am not sure of an ideal price, so please post what you feel is fair. At the end I am looking for them to give a brief explanation on the features and the important choices they have made in the mock up for the sake of CRO. I am expecting it to look nice while also holding high conversion. If you feel that some of these elements should be moved below or above the fold, feel free to do so, since you would have more experience than I do. I do not have pictures, so feel free to use stock pictures.
I am new to this, so I am looking for someone with some patience.
