Hi, I have a setup already like the survey you linked to, which I can amend to include the questions you want to ask. All I would need are the questions, and what you would like done with the answers... I assume you would like them inserted into a database and then to be emailed every time someone completes the survey? I do this sort of thing on a daily basis as a web developer, so I can have it ready in a matter of hours, database included and all nicely formatted in the design. I would also modularise the code, so that you could include it on any website you wanted, or even resell it :-). Looking forward to hearing from you. Kind regards, Will

I would be happy to produce a functional survey with data gathering and reporting as agreed upon in setting the project scope. I have generated many survey engines, from a simple sign up form for a dentist twenty years ago to industry sponsored surveys in the hospitality and insurance industries. I feel $266 is a reasonable ball park for the kind of survey you suggest. I would use the questions provided by you to implement a PHP survey in your existing website. I am able to use your current template or otherwise match your websites look. Depending on your needs and system architecture I would save the filled out surveys in a MySQL database or csv file. Or email them to you per completion. If the data is stored on the server (MySQL or CSV) I would create an admin side capable of generating response reports. summary of completions by month and the ability to download all, or monthly, responses in a spreadsheet. I am happy to discuss your exact needs with you to agree on a scope and possibly adjust the quote if appropriate.
