Custom Fax Order Form .PSD file

I need a custom order form made up so customers can fax me their orders. The form, which must be made with adobe photoshop (.psd file) must include: logo area (and basics, such as company name, address, phone number, etc.) Customer info: name, address, e-mail, telephone product order info (quantity, item #, item/description, price ea., total price) subtotal shipping charge sales tax box (MI only) grand total of order credit card info (visa, mastercard, american express, discover) gathering info including CC number, ex. date, and CVV code (with instructions on how to get CVV code) billing address (if different from ship to) card holder signature policies section shipping charges section payment methods section misc. things you can think of Form must fit in 1 page and have at least 15 different items to enter for the customer. By PMing me and showing me what you have made for this, that will only increase your odds of being the winner. File submitted at that point can be a low res so I don't take it and run, but again, if you submit a picture of form before I pick winner, your submission will be that much stronger.
