Custom USB hub PCB

We need a custom USB PCB. This will not be anything special: * 7 downstream ports, 1 upstream, working to USB 2.0 at full (480mbps) speed * optional power header connector * LEDs per downstream port indicating status The main need for customisation is to get the ports located where we want them, so I mainly see this as a PCB layout problem: using a reference USB hub design would be fine. We have a format design in PDF format to form the basis of the design in terms of port placement, with specific measurements. We hope to have the PCBs made relatively cheaply, so design considerations in terms of cost would be welcome - in terms of both choice of components, and circuit choices (static protection, etc.). The outputs of this project would simply be CAD files (Gerber or similar) and part lists etc. that we are then able to use to fabricate the boards themselves. Suggestions for manufacturers in the UK/EU also welcome.
