Create an introduction function

We are creating a series of training videos and have already made up our introduction, but am now looking to have the starter information section created. This section will have two parts, one being an introduction to the presenter/voice over (I have attached a caricature of myself which we have for every staff member and would be used) and the other should comprise the What, When, Who, Where, How Long and Why for this training - which will be different for each training video. (We envisage this introduction would be controlled to fade in while the voice over reads/elaborates). We don't believe what will be provided in this project will be a video, but rather an easily editable document that we can change for each training video. We want it to be in line with our current introductory video (see attached) which will open the video and then be followed by this content and then by the training. I have also included a screenshot of the form we create the content that will be used (but keep in mind this is just what we use - we want something pretty). We are sick of seeing the standard PowerPoint style introductions, they are not conducive to learning and in most instances are just ugly, so some artistic licence is expected, but please remember that we need to be able to interact with the template for every video. What I envisage is a pretty document template that we can add the content to (file format is not important so long as we can use it - and if it can do transitions so we can just do a screen recording that would be great).
