Create a Survey in Moodle

Thanks for posting an interesting project. In fact, I have experience in development of custom Survey module in Moodle. Please note, 70% of the projects I am doing are of Moodle. Our module will be installable, so you will be able to install in as many moodle sites you want. Kindly check feedback of clients in moodle projects. I can give export feature also. In ms-word or pdf , whatever you want.

Hi, I've already done the same work for a few companies here in Italy. Yes, I can confirm that I'd use the Feedback module which also allows to create a template and to import/export the template (I don't think it's possible to link all sites to one single survey without a strong customization). In the end of the survey you can export the results through an Excel file. As for the questions in the link you posted, the Feedback module fully supports all the types of questions you require (Textbox answer, Multiple choice by checkbox and dropdown), so I don't see specific problems for the task you are asking. What versions are the Moodle installations on the interested sites? You say you have access to one Moodle site, but you need the same survey available on 6 or 7 different Moodle sites... can you please clarify? I have experience in installing and maintaining moodle platform in my university when i was doing post graduate studies. I worked as a software developer in Faculty of Medicine and Health Sciences when i was doing my post graduate studies during year 2009 to 2011. I was an administrator to the moodle platform in Faculty of Medicine and Health Sciences. I create courses, invite lecturers, professors, import students, create quiz, create mid term test, create assignment, upload lecture notes, create a new semester, invite students into the courses, and etc. The students and course instructor can chat among each other as well. Currently I am graduated, I no longer helping my professor, because I graduated, and find a job in other organization. I am glad if you choose me. I use to work with Moodle from 1.8 to 2.6 and can insert a survey with the same questions as on your link. We can do it with the feedback feature, indeed. We could export the survey in xml format to import it in other Moodle sites.

As you pointed out the survey should be in Moodle, but is it one survery that all Moodle websites are connected to? Or every Moodle website has it's own survery? This makes the difference in the results and you want to manage them. In both cases you should have access to all Moodle websites.
