Please look at the attachment for an example of how the completed page should look. Fonts and colors are not important since I will apply CSS styles to it after completion.
The company delivers lawn ornaments to homes such as pink flamingos, storks, etc. for special occasions like 40th birthday, newborns, graduations, etc. The deliveries are all made by 6:00AM on the drop-off date and picked-up after 7:30 PM on the pick-up date, so there is no need to record a time of day.
This project is a web form that writes entries from all form fields to a MySQL database and populates a calendar on the page in real time. (No page refresh) The form as shown will be used by office personnel when taking phone orders. It will not be on the public website.
The only entries needed on the calendar are the drop-offs and pick-ups scheduled for each day. Each entry must show the Customer Last name, Street address and City on the left side of the calendar. The right side will show the items. Every Drop-off will have a corresponding Pick-up so each time the form is completed two entries are made on the calendar.
The calendar must retain all of the reservations entered through the form. When the page opens, the current week will be visible. The user must also be able to scroll forward to schedule reservations in the future or backward to review reservations from previous weeks.
Date-pickers, something like in the sample page, rather than text boxes or menus should be used to choose the dates.
The 4-line Message Sign must be all upper case letters with a maximum of 12 characters including spaces on each line.
The form should be no more than 900 pixels wide. Length can be whatever is required.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
