Customization of WP-Members - repost

Customization of WP-Members plugin on current WP website. 1. Add custom fields to member registration. 2. Generate a form on a members only page so registered members can fill out the details. Once the form is submitted a wordpress post is created and published with their content from the form. Fields on the form include, Name (Title of post), Photo File or Video Embed code (content), Description (content), date (post date), location (cat), tag (tag) Username (editor) 3. If a user uploads a video an advert (pre load) is required to play on before their video. JW Player could be installed to play the embed code and do an advert before. I am open to other solutions. The whole process needs to be automated. Final code should be packaged as a plugin and be compatible with WP-Members and the latest WP. Simple update to an recently developed site.
