1. An image of what the report will look like. Try and make the report look as close as possible to the report I have sent.
2. Project to work from. You will see a form called Incident_New. It contains all the data to go on the report.
3. You will notice that many of the checkbox sections on the report have now been converted to dropdown boxes on the form.
Do not include the checkboxes only what was selected in the dropdown

TYPE OF INCIDENT: Fall.
4. You will notice that when you select Person Involved you get different controls showing make sure you account for this on the report.

6. THere are 3 boxes Details, Actions Taken and Followup. Make sure that he report will stretch to account for these fields as there may be a lot of text in them
