database & web interface

I need a database and web-based user interface setup. Provider will be given access details (ftp & cpanel) to my new website. A simple database will need to be setup. Then a web based user interface will need to be written to enable me to write to the database and extract information from it. Database records only have 3 fields. My main need is that data can be written and extracted FAST. Database will often be over 200,000 records so speed is essential. Inputted data will need to be checked to prevent duplication in the database. There will definitely be further ongoing work if this goes well.
