Database, Form and Printable Results Page

I have a .doc file at the moment, and I need to construct a system where the user can fill out a form online, (which I can create) and the info goes in to a database, (I will create a database, however I will need the insert script) and then this will need to look like the orignal forms provided. I need the staff to be able to print off the users information to fit in to the document layout.. This will also need to generate an email to a user, that looks like: to info@...xx... You have had a new registration. Click here to print the registration [login to view URL] This is for a volunteer charity, so a bid as low as possible would be great. I have MSN for easy access, and will pay via GAF so we can leave feedback.
