I need a copy of this middle part, but with a map of poland:

here is a map as you see that counties.... this must be clean rebuild without cities and without the other countrys... i need only poland: [url removed, login to view]
I also need the counties with numbers and on right hand i need the numbers listed with county name...
e.g.: 1) Pomorskie 2)Zachodnio-Pomorski
i want source data to do small gfx changes by myself instead of asking 200 times.
on the map hover a div should be shown with more information (this could be a placeholder at the moment)
see attached picture for layout of blank html page. no flash wanted, need jquery if needed.
