C# application to create dot (MS Word) file, then populate and print several filled templates

I need to create a word document template (dot file) which is going to be populated from records from an oracle database table. The application is a C# forms application. The application needs create the template first, then populate several dot files and print them without saving them. Let’s say there are 15 records in the db table. Then the application needs to take the first row, populate the word template, print it, and then continue with the other 14 records. I need the template to be created also. Also, there are some fields in the template which won’t be known before hand, i.e. the fields need to be dynamically created (this is why the C# application needs to create the template and it can't be created manually once). In my attached dot example file, you will see on the right there are a set of fields called BoolField1-7. These fields come from the database and until you run the application you won’t know how many fields there will be (you may have from BoolField1-3 or BoolField1-200). These fields need to be created dynamically and also populated with true/false for each one. These fields can be created as either bookmarks or as template fields (please specify how it is done). I need to be able to modify the template as well as the C# forms application to add in my own custom fields, so I am expecting a well documented program and a few instructions how to add my own fields to the document. I don’t need any oracle scripts, just assume all the tables / data already exists. Please let me know if this needs more clarification. The documents don't need to be saved for anything.....I just need to print them and discard the soft-copy. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
