Wordpress Plugin (UNIQUE) EXPORT AS PDF

I need a wordpress plugin made. The website offers cards to its members. (Printed ID Cards) Much like a drivers license card. ADMIN AREA: The plugin needs to export user data onto a card design detailed below. Exported as a PDF Preferably. Unique member id (6 digit number) Photo of user First & Last Name of user Date of Birth of User. It needs to display this info on top of a card design. (Template, Image) So it can be printed off and used as an ID card. The Card designs need an area in the admin to be uploaded. (changed) Front and back. The card designs should be able to be assigned a Group. So multiple designs can be used and assigned to different membership levels. -- The site will be using S2Membership WP plugin. So it has different membership levels. -- USER SIDE: There needs to be a page for the users to add their data when signing up.
