Clone A Site

We need a site similar to this - approvalwithbadcredit /./ com This site needs to work with PHPList ([login to view URL]) as far as initial collection of customer email. This one "subscribe" page is all that is needed. Pages that customer is forwarded to after collection of email will be done on our end. Placeholder graphics, landing pages, and copy will be suplied. Here is what we need the subscribe page to do: -user selects 1 of 7 options from radio buttons(mandatory) -user enters email address (mandatory) -user checks TOS box (mandatory) -user clicks "Submit" email button and is redirected to appropriate landing page from initial option selected. See "approvalwithbadcredit /./ com" for what subscribe page should look like. Email confirmatiom will be turned off in phplist allowing the visitor/user to be redirected to appropriate landing page without having to answer a confirmation email validating their address.
