Digg style login (Modal Window)

I am looking for a Digg style login using a modal window or lightbox. The login should be exactly like digg when clicked a modal window pops up and the following information needs to be inputted to login. 1. Username 2. Password 3. Remember Me (check Box) 4. Login Button 5. Lost password? (When clicked it acts similar to the digg's lost password and you have to input email and clock on the retrieve button) 6. create a new account (link at the button) When the user clicks on create a new account they are direct to the sign up form like [login to view URL] (you get the idea)everything that is needed is what I need for user information with the added addition of first and last name containers. All information for the login and registratopn is part of the site for login and registering now but in a static page. Like mentioned above use [login to view URL] login form and registration and style for design information and reference. I would like this done professionally and quickly. Please reference any similar work with your bid or PM. No database tables will need to be created all that will be needed is a fancy eye pleasing modal window and everything else should be there to be inputted into the code of the modal window.
