Need a PHP web application made which collects data entered by a user and adds to a database.
Only have a budget of up to $120 SO ANY HIGHER BIDS WILL BE IGNORED. Good project for someone to take their time and work on for a few weeks.
The system will allow the user to search for a term to see if it already exists in the database and alerts the user if it does or doesn't already exist in the database. If it does exist, information is displayed so the user can decide if they should proceed. The user will be able to enter free text and select some drop downs.
There will be a stats page which will track the total time spent and average number of records processed.
The system will also have an Admin section where an Admin can create / delete user accounts and then export the database into csv all from the admin page.
The database may become very large 1 + records so considerations need to be made to accommodate for this. The application needs to be able to be accessed and used by more than one concurrent user at any given time on any browser e.g. I.E, Chrome, Firefox.

Application to be installed on shared host. Application remains property of mine i.e. source code handed over.
Domain for the website will be given to you and you will need to setup.
