Database, HTTP Post, Form Built

I'm looking for someone to build out my long form for me. [login to view URL] We have the basic page built out, I need the year, make, model associated to a database that you create. I have a large csv file with list of years, makes, and models that I'll need you to build out a database for so that the form can call back to that database. I need the previous page edited and also a 2nd version of the page so I can split test what converts better. Currently the page has a static form for 2 Vehicles, but I need the 2nd fields for the vehicle removed and added ONLY when the user needs to add another vehicle to try and insure it. For example: [login to view URL] you can see the green button that is done by javascript, the user is allowed to add more fields when they click that button. For the second page, version two I want the fields each section to appear as they complete each section. For example on this page [login to view URL] you'll see there are sections, Vehicles, Drivers, Additional Information, and Primary Driver Contact Information. I want only Vehicles to appear when they load the page as they fill out and complete all the fields for Vehicle, Drivers section will auto appear, as they complete Drivers section, Additional Information section will appear, so and so forth. An example of this is here: [login to view URL] click on give it a whirl and you can see the feature I'm talking about, however they are breaking it out with individual fields popping up, i need sections popping instead of individual fields. Also for both pages I need all the data fields that are on this page to post out to a 3rd party database via XML or HTTP POST. HTTP POST will work good as it seems to be more compatible.
