Database and script setup

I am need of a very simple project that I am too busy to complete. I need a script setup on my server along with a very small database. I will provide clear setup instructions in writing along with any other things that you may need to get the project completed. There will be very little programming needed. The script is already done. Minor php programming may be needed if any. Set it up test it out.
