Corrections to JMail Script

We received notice from our web host provider that "Effective, October 15th, 2004 we will no longer be supporting Bamboo ActiveX component." Unfortunately, this is what three of my forms (four forms) were using. I've since had them coverted to JMail, however, the web host provider tells me that there are "coding errors" in the script, so I'm in need of someone able to correct these errors. The major portion of these mail forms is done in VBscript (which I believe is still okay.) I've included a sample of the script that needs revising and would send the provider's instructions to the person accepting this bid. I don't believe that much needs to be changed, but since this is something I'm not familiar with, I'm not sure which small part needs changing. Anyway, here's a portion of the current script.
