Building MySQL database and front end

Hi – I’m looking for someone with MySQL and PHP knowledge to build a database for me. It’s got to be on my server and password controlled, with 2 levels of access, users and admin. Inside there is going to be a couple of forms and a whole load (about 20) fields which users are going to fill in. They must be able to create new records and edit current records. It is also important that the fields are dependent on an initial choice – a drop down option. The forms should be similar in format to an access database, and it will have a contact history where users will input who they contacted, etc etc which will be time and date stamped. There will also be a big search page with all the variables so users can find specific records. The admin section will have a section on registering new users, changing drop down values, and also doing some reporting. I would like quotes from people with MySQL and PHP experience, and who have built similar style databases that they can show me.
