Please see the attached PDF file. It comprises of scanned pages having contact info of businesses. I want all contact info in that file to be entered in MS Excel in following fields (i.e. each record will have the following):

[login to view URL] (2 letter abbreviation e.g. CA, TX etc)

[login to view URL]
[login to view URL] (if and where available)
[login to view URL] (if and where available)

**There are 1200 or so records / business contact info.**
Hence, I am willing to pay upto **4 cents** per record.
**Accuracy, timeliness and consistency in entry** are of importance (in that order).
If the task is completed within 4 days, I guarantee a bonus and an invitation for the next data entry job.

## Deliverables
1) Complete and fully-functional file in MS Excel 2000 or above.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
