Access and ASP forms converted to MySQL and PHP

We are a 501(c) non profit that matches volunteer drivers with elderly that are in need of transportation to and from their doctors appointments. We currently have an online form based application that collects volunteer driver information and client information then allows us to match drivers to clients for specific trips that are either one time or reoccurring. The current application is ASP and MS Access based and we would like to convert it to MySQL and PHP. You can either convert what we have or create it from scratch, but we have a limited budget as we are solely dependent on donations. We can provide the winning bidder with a receipt for the discount you have provided to us that can be used as a tax write-off, so if you want to make a little money and get a BIG deduction on your 2012 taxes, we can help! I have attached several screens from the application. There isn't really any logic behind this app, just forms you fill out and reports that can be run (i.e. queries). What I have described above are the basics of what we need, and there are several options on the existing menu that don't get used or could be consolidated into a more dynamic option. The new website will be developed on Joomla, so if you have experience with that CMS, it would be a big help as authentication integration is desired but not a requirement. I also have some limited capabilities to assist with this project.
